Fast Math Routines

A collection of good fast math routines including a fast table-based square root, and FloatToInt().  Never cast with (int) unless you understand why you shouldn't!!

4/00  New contribution of FP_INV2(r,p) in assembly thanks to Vincent Van Eeckhout
Also a new fast 3D point-in-arbitrary-cylinder test.
